You take over the Klinikum am Rhein — a small clinic in the German Rhineland — and grow it: fit out treatment rooms, hire staff, buy equipment, license new specialities. Patients arrive with different cases — and with different amounts of patience.
The appeal is in the planning. A doctor costs real money per hour, an empty room still costs rent, and a patient who waits too long simply leaves. Your reputation decides how many people turn up at all.

Rostering is the heart of the game. Your hospital runs around the clock in three shifts and each shift needs its own staff. A doctor assigned to the early shift is not available at night — those rooms then sit empty even though you built them.
Staff is your biggest cost. A consultant costs far more than a nurse but handles cases nobody else can. Hire expensive too early and you slide into the red.
Patients, waiting time and reputation
Patients keep arriving with a case you can treat — or cannot. Anyone who waits too long leaves untreated and your reputation suffers. A good reputation brings more patients, more patients need more staff. That is the screw you turn all game long.
The canteen — a link to the farm
Once licensed you have to feed your patients. You can buy from a caterer — or sign a standing contract with a farm player. Eggs and milk from there are noticeably cheaper, and full coverage lifts your reputation as well.
Free, with no real money
The hospital is a browser game: open it, play, install nothing. And there is no shortcut you can buy. No subscription, no premium currency, no ads. If money is tight early on the bank will lend you some — but you still have to pay it back.

Why can I not assign my second doctor to a room?
Because every shift has its own occupancy. Check whether you are editing the early, late or night shift — the slot may be taken in one and free in another.
Does the hospital keep running when I close the tab?
Yes, it runs in real time. Everything that happened while you were away is calculated when you come back.
Can I go bankrupt?
There is no real bankruptcy, but you can go into the red. Then it gets tight: the bank charges interest and without money you cannot hire anyone.
